Religious and medical exception applications available for the employee vaccine requirement

The Office of AA/EEO and Title IX, which is responsible for processing all requests for religious and medical exemptions to the employee vaccine requirement, strongly encourages all employees to submit their request forms no later than Dec. 8, 2021, to receive full consideration.
