Updated COVID-19 Interest Form available to employees and students

As more vaccine becomes available, the University will continue to open more appointments for employees and students. Faculty, staff and currently enrolled students who have NOT submitted a form indicating an interest in receiving the Covid-19 vaccine through the university can do so today by completing this online form or scanning the QR code on this page. The updated form does not require risk factors and simply allows the campus community to be added to the waiting list. Those who submitted the previous form are already on the waiting list and do not have to complete a new form. As the university moves through later phases of the vaccine prioritization plan, individuals will be prioritized based on age, from oldest to youngest.
